Our Patient Health Navigators work closely with healthcare providers to identify patients who require additional support or resources. They provide education on chronic conditions, facilitate communication between patients and healthcare providers, and assist patients in accessing community resources. This role requires excellent communication skills, empathy, and a strong commitment to delivering patient-centered care.

About Us

Sea Mar Community Health Centers is a Federally Qualified Health Center (FQHC) with over 90 medical, dental, and behavioral health clinics across Washington State. We are a community-based organization that provides comprehensive healthcare services to diverse populations, including those who are underserved and marginalized.

Responsibilities

  • Schedule appointments and navigate patients through the healthcare system
  • Provide education on chronic conditions and facilitate communication between patients and healthcare providers
  • Assist patients in accessing community resources and services
  • Participate in mandatory trainings and service projects

Requirements

  • High school diploma or equivalent required; Bachelor's degree preferred
  • Excellent communication and interpersonal skills
  • Ability to work in a fast-paced environment and prioritize multiple tasks
