About the Role

We are seeking a highly skilled Care Manager Professional to join our team at Gateway CSB PEO LLC.

This role involves performing assessments, treatment planning, and discharge planning under general supervision. You will work closely with clients, providing counseling services and coordinating with an interdisciplinary team to ensure effective treatment outcomes.

The ideal candidate will have a Bachelor's degree in a helping field and relevant experience in the area of assignment. A strong knowledge of individual and group therapy techniques, diagnostic impressions, and client advocacy systems is required.

The salary for this position is approximately $60,000 - $80,000 per annum, depending on location and experience.

Key Responsibilities
  • Conduct intake, screening, assessment, and referral of clients as appropriate.
  • Provide clinical assessments by gathering relevant information to identify problems and determine eligibility criteria.
  • Develop person-centered plans for service users.
  • Evaluate, facilitate, and monitor services and client progress in relation to established goals and objectives.
Requirements
  • Bachelor's degree in a helping field from an accredited program.
  • Associate license preferred in the area of assignment.
  • Strong knowledge of individual and group therapy techniques.
  • Ability to work and communicate effectively with diverse populations.
