We are in search of an experienced and driven RN Care Coordinator to assist us in achieving our objectives.

We will equip you with the necessary tools and resources to thrive within our organization and advance your nursing career.

Your responsibilities will include coordinating services throughout the continuum of care, ensuring a balance between high-quality patient care and the efficient use of resources.

You will conduct concurrent clinical reviews to assess the appropriateness of admissions and the ongoing necessity for care in the acute hospital environment.

Collaboration and communication among the patient/family and the entire healthcare team regarding the patient's care plan and post-discharge needs will be supported by you, facilitating the required services.

You will perform telephonic clinical evaluations with insurance claims personnel to assist in the reimbursement process for patient care services.

Identifying quality and/or risk issues and reporting them to the appropriate individual or department will also be part of your role.

Graduation from an Accredited School of Nursing is required; proficiency in assessing, planning, and managing patient care delivery is expected, with a minimum of 2 years of clinical nursing experience in an acute care setting.

Prior experience in utilization, quality, discharge planning, and/or case management is highly preferred.
