Job Summary

We are seeking a skilled Licensed Practical Nurse to join our team at Commonwealth Care Alliance. As a LPN, you will play a vital role in providing high-quality patient care and support in our clinic setting.

Key Responsibilities
  • Manage patient flow through the clinic, obtaining pertinent clinical information and preparing patients for examination.
  • Implement patient treatment plans and therapies under the supervision of a registered nurse.
  • Assist patients with dressing, undressing, and positioning, as well as perform phlebotomy, give immunizations, and obtain specimen samples.
  • Document patient history, standardized screening assessments, and vital signs.
  • Collaborate with the care team to assess patient needs, plan and modify care, and implement treatment plans.
  • Participate in administrative patient care functions, including scheduling appointments and completing preauthorization forms.
  • Provide clinical care to members via telehealth technologies, as needed.
Requirements
  • High School Diploma or equivalent required; Associate degree preferred.
  • Minimum 2 years of clinical experience; current LPN license to practice in Massachusetts required.
  • Current CPR certification and willingness to learn telehealth technologies.
Working Conditions

This is a standard clinical environment with standard clinical equipment. English is the primary language spoken, but bi- or multi-language skills are highly desired.
