VNA Health Group is seeking a skilled and compassionate Registered Nurse to provide high-quality patient care in the comfort of their own homes.

Job Summary

We are looking for a dedicated and experienced RN to assess patient needs, develop and implement individualized care plans, and collaborate with healthcare teams to ensure seamless care transitions.

Responsibilities
  • Conduct thorough patient assessments to identify needs and develop effective care plans.
  • Collaborate with healthcare teams, including physicians, therapists, and social workers, to ensure comprehensive care.
  • Provide direct patient care, including wound care, medication management, and education.
  • Monitor patient progress, adjust care plans as needed, and communicate with healthcare teams to ensure continuity of care.
  • Document patient information, including assessments, care plans, and progress notes, in a timely and accurate manner.
Requirements
  • Current RN licensure in the State of Ohio.
  • 1-2 years of experience in home health care or medical-surgical nursing.
  • Strong assessment and critical thinking skills.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
