VNA Health Group is seeking a skilled and compassionate Registered Nurse (RN) to fill a Home Health Manager position in Warrensville Heights, Ohio.

Job Summary:

The RN Case Manager will provide direct or indirect comprehensive nursing care to patients in their homes, working closely with healthcare providers and third-party organizations to ensure seamless care coordination.

Responsibilities:

  • Utilize the nursing process to collect data, plan, and evaluate nursing care for patients.
  • Demonstrate expertise in implementing prevention, therapeutic, and rehabilitative nursing care.
  • Communicate effectively with healthcare providers, patients, and families to ensure comprehensive care.
  • Maintain awareness of patient comfort and safety needs, observing and recording significant conditions and reactions.
  • Respond to life-saving situations based on nursing standards, policies, and protocols.

Qualifications:

  • 1-2 years' experience in Med Surg or hospital setting.
  • Completion of an accredited RN education program with a diploma, Associate's or Bachelor's degree.
  • Valid driver's license and CPR certification required.
  • Bi-lingual a plus.
