Job Summary:

We are looking for a dedicated and detail-oriented RN to provide high-quality patient care in the comfort of their own homes. As a Home Health RN Case Manager, you will be responsible for assessing patients' needs, developing and implementing individualized care plans, and collaborating with healthcare teams to ensure seamless care transitions.

Responsibilities:
  • Conduct thorough assessments of patients' physical and emotional needs
  • Develop and implement comprehensive care plans that address patients' unique needs and goals
  • Collaborate with healthcare teams, including physicians, therapists, and social workers, to ensure coordinated care
  • Provide education and support to patients and their families to promote self-care and independence
  • Monitor patients' progress and adjust care plans as needed
Requirements:
  • 1-2 years of experience in Med Surg or hospital setting, preferred
  • Completion of an accredited RN education program with a diploma, Associate's or Bachelor's degree
  • Valid driver's license and CPR certification
  • Ability to work independently with minimal supervision and maintain strict confidentiality
