Job Title: Pharmacist (Pool)

We are seeking a skilled and dedicated Pharmacist to join our team at Centers for Independence. As a Pharmacist (Pool), you will play a vital role in providing high-quality pharmaceutical services to our clients.

Job Summary:

The successful candidate will be responsible for all aspects of pharmaceutical dispensing, including interpreting prescriptions, checking for drug interactions and allergies, and providing clinical support to clients. You will work closely with our healthcare team to deliver quality and cost-effective drug therapy to patients.

Key Responsibilities:
  • Interpret physicians' prescriptions and enter prescription and patient data into our computer system.
  • Check for drug interactions and allergies, and provide clinical support to clients.
  • Provide Pharmacy Technician prescription information and supervise filling of same.
  • Verify prescriptions after filled by Pharmacy Technician.
  • Counsel case managers and clients over the phone and in person, conveying pertinent information regarding use of medications and potential interactions.
  • Consult with other health care professionals and conduct research to ensure that all services are in the best interest of the patient and conform to established professional standards and applicable state laws.
  • Compare product prices with competitors to gauge competitiveness and profitability.
  • Review inventory on a daily basis, place orders for medication as necessary, and maintain appropriate levels of supplies.
  • Encourage teamwork through cooperative interactions with co-workers.
  • Attend and participate in staff meetings, in-service training seminars, and conferences as required.
  • Work in partnership with the quality department to develop appropriate measures in support of program outcomes.
  • Participate in department quality planning activities, including continuous improvement, logic models, performance metrics, etc.
  • Perform special assignments and/or projects as assigned.
Requirements:
  • Bachelor's or PharmD degree in Pharmacy from an accredited college or university.
  • Minimum of 2 years' experience working in a healthcare setting; experience working in a long-term care or mail order facility is preferred.
  • Registered Pharmacist license with the State of Wisconsin required.
  • High attention to detail and strong mathematical skills.
  • Familiarity with Microsoft Word and Excel.
  • Professional workplace appearance and conduct; friendliness and courtesy to clients, coworkers, and vendors.
Working Conditions:

This position works in a fast-paced office setting and frequently utilizes various office equipment, including a personal computer, multi-line telephone, fax machine, and office copier. On occasion, you may be required to push/pull/lift/move objects weighing at least 10 pounds, stand for long periods of time, and perform repetitive fine motor skill-based tasks.
