Job Title: Hospice Care Professional

About the Role:

We are seeking a skilled and compassionate Hospice Care Professional to join our team at PruittHealth. As a Hospice Care Professional, you will play a vital role in providing high-quality care to patients with terminal illnesses.

Your primary responsibility will be to provide comprehensive nursing care to patients in their homes or in hospice facilities. You will work closely with interdisciplinary teams to develop and implement individualized care plans that meet the unique needs of each patient.

  • Provide direct patient care, including wound management, pain control, and symptom management.
  • Conduct regular assessments to identify patient needs and update care plans accordingly.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
