About the Role:

  • Develop and implement individualized plans of care for patients and families, in collaboration with the interdisciplinary care team.
  • Assume full responsibility for professional management of assigned hospice patients' care related to their terminal illness.
  • Work closely with attending physicians and other healthcare professionals to ensure seamless care transitions.
  • Supervise and educate certified nurse aides and other caregivers to ensure optimal patient outcomes.
  • Participate in performance improvement initiatives and contribute to the development of policies and procedures.

Requirements:

  • Current, active, and unrestricted Registered Nurse (RN) licensure in the state of practice.
  • Excellent communication and organizational skills, with the ability to work effectively in a fast-paced environment.
  • Strong knowledge of hospice regulations, laws, and best practices.
  • Able to maintain confidentiality and handle sensitive information with discretion.
  • Flexibility to work a variety of shifts, including evenings, weekends, and holidays.

Benefits and Compensation:

We offer a competitive salary range of $90,000 - $115,000 per year, based on experience, plus a comprehensive benefits package, including health, dental, and vision insurance, 401(k) matching, and paid time off.
