Clinical Care Transition Educator Job Description
The Clinical Care Transition Educator is a key member of the Option Care Health team, responsible for ensuring the successful transition of patients to our care delivery model. This role requires a unique blend of clinical expertise, communication skills, and business acumen.
Main Responsibilities
- Develop and deliver clinical education programs to patients, caregivers, and healthcare professionals to ensure seamless transitions to our care delivery model.
- Collaborate with the sales team to identify and address clinical transition barriers, driving growth in patient transitions and improving overall patient outcomes.
- Conduct thorough patient assessments and evaluations to determine viability for our care delivery model.
- Maintain confidentiality and adhere to strict guidelines for safeguarding patient and proprietary information.
Requirements
- Active and unrestricted Registered Nurse (RN) license
- Minimum 2 years of experience in the healthcare industry
- Excellent communication and interpersonal skills
- Ability to work in a fast-paced environment and adapt to changing priorities
