As a key member of our healthcare team, you will play a critical role in ensuring that patients living with HIV receive the care and support they need to achieve viral suppression.
Key Responsibilities- Conduct Outreach and Intake: Develop and implement outreach strategies to engage patients in care, including those who have fallen out of care.
- Assess and Develop Care Plans: Conduct initial assessments and reassessments to determine patient needs and develop comprehensive care plans.
- Provide Medical Case Management: Manage a caseload of patients and provide medical case management services, including eligibility screening and intakes.
- Collaborate with Healthcare Providers: Work closely with healthcare providers to ensure patients receive timely and appropriate referrals to onsite and offsite service providers.
- Document Patient Encounters: Document all patient encounters and attempts to contact patients in progress notes.
- Participate in Quality Assurance Activities: Participate in quality assurance and performance improvement activities to ensure high-quality patient care.
- Education: High school diploma or equivalent required; bachelor's degree in human services or related field preferred.
- Experience: One to five years of experience working in HIV prevention, treatment, and adherence; minimum of three years of job-related experience.
- Skills: Ability to use verbal and nonverbal calming techniques; proficiency with Microsoft Office software; excellent communication and interpersonal skills.
The Retention and Adherence Specialist will work in a fast-paced healthcare environment, requiring the ability to multitask and prioritize tasks effectively. The position may require prolonged periods of standing, walking, talking, sitting, hearing, and bending throughout the workday.
