Nursing Care Coordinator

The Clinical Coordinator plays a crucial role within the healthcare team, reporting directly to the Director of Nursing. This position is essential in ensuring the highest standards of care and enhancing the quality of life for residents by proactively addressing clinical challenges and delivering effective training to nursing staff.
Qualifications:
Applicants must be graduates of an accredited nursing program and hold a valid RN license in Alabama. A minimum of two years of nursing experience in a clinical environment is required, along with at least one year in a supervisory role within a long-term care setting.
Candidates should exhibit the necessary knowledge and skills to provide care tailored to the unique needs of the residents. Strong leadership and supervisory capabilities are essential, along with a commitment to teamwork and support for nursing personnel.
Additionally, the ability to plan, organize, develop, implement, and interpret the necessary programs, goals, objectives, policies, and procedures to ensure quality care is vital.
Key Responsibilities:
- Ensure the availability and proper setup of medical equipment in residents' rooms prior to their admission.
- Conduct assessments for new admissions and readmissions upon their arrival.
- Perform daily rounds with Nursing Team Leaders to monitor any changes in residents' conditions.
- Review nursing documentation to ensure accurate records of treatments, medications, and conditions.
- Manage the admission, transfer, and discharge processes for residents as required.
- Oversee residents' treatments and medications to guarantee appropriate care is being provided.
- Participate in department head and nursing management meetings as directed.
