Job Summary

The RN Care Manager is a key member of our interdisciplinary team at Life Care Center of Coos Bay. This role is responsible for coordinating patient care and services, promoting quality care through a collaborative process, and ensuring a safe transition from our facility to the next provider or care setting.

Responsibilities
  • Coordination of Care: Work with patients, families, care teams, payers, and external entities to promote quality care and ensure a smooth transition to the next care setting.
  • Case Management: Utilize expertise in case management to coordinate patient care, monitor services, and evaluate patient needs.
  • Communication: Develop and maintain effective communication with patients, families, care teams, and external entities to ensure seamless care coordination.
  • Education and Training: Provide education and training to patients, families, and care teams on patient care and services.
  • Quality Improvement: Participate in quality improvement initiatives to enhance patient care and services.
Requirements
  • Education: Nursing diploma (associate's or bachelor's degree in nursing)
  • Licensure: Currently licensed/registered in applicable State. Must maintain an active Registered Nurse (RN) license in good standing throughout employment.
  • Experience: One (1) year of clinical experience in post-acute care setting preferred
  • Certifications: Certified Case Manager (CCM) or Board Certification in Nursing Case Management (RN BC) preferred
