Position: HOSPICE LICENSED PRACTICAL NURSE (LPN)

Employment Type: Full Time
Sign-on Bonus: $5,000

Location: Durham & Burlington

Role Overview:

The Hospice Licensed Practical Nurse will:

  • Deliver direct patient care, utilizing a diverse skill set and ensuring the necessary coordination of services.
  • Complete all required documentation in accordance with established policies, procedures, and regulations.
  • Implement the care plan tailored to the patient's diagnosis and condition as developed by the Registered Nurse.
  • Collaborate with the Registered Nurse, Patient Care Coordinator, and other agency staff to enhance patient care, while engaging with the patient, family, community agencies, and physicians.
  • Receive and transcribe physician's orders as instructed.
  • Forward all clinical notes, progress reports, and summaries to the Hospice Registered Nurse for coordinated care.
  • Participate in Interdisciplinary Team (IDT) meetings.

Qualifications:

  • Completion of nursing education from an accredited nursing school.
  • Current LPN licensure in the relevant state and a minimum of one year of clinical nursing experience, preferably in an acute care medical/surgical environment.
  • Ability to independently manage workload and deliver effective healthcare services to hospice patients.
  • Capability to ensure appropriate patient care under the supervision of the Hospice Registered Nurse, with thorough, accurate, and timely documentation.
  • Strong communication skills, both verbal and written.
  • Current CPR certification.
