Hospice Licensed Practical Nurse

At HealthEcareers - Client, we are dedicated to providing exceptional care to our patients and their families. We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our hospice team.

Job Summary

We are looking for an experienced LPN to provide direct patient care to our hospice patients. The successful candidate will have a strong background in nursing, excellent communication skills, and the ability to work independently.

Responsibilities
  • Provide direct patient care, including administering medications, wound care, and vital sign monitoring.
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team.
  • Coordinate hospice services with the patient, family, and healthcare team to ensure seamless care.
  • Document patient progress and communicate effectively with the healthcare team.
  • Participate in IDT meetings to discuss patient care and develop plans for improvement.
Requirements
  • Current LPN licensure in the state of employment.
  • One year of clinical nursing experience, preferably in an acute care setting.
  • Ability to work independently and as part of a team.
  • Excellent communication and documentation skills.
  • CPR certification.
