Job Summary:

RN position oversees Home Health, Hospice, Infusion, and Advanced Care at Home referrals for Rex and Holly Springs Hospitals and Rex Skilled Nursing Facilities. The role educates patients and families on administering infusion therapy, navigating home services, and working with Case Managers, Providers, and referral sources to ensure efficient care transfers.

The Subject Matter expert for all services provided by Continuing Care Services, this position requires patient and provider teaching, excellent computer skills, and home health or hospice experience.

Responsibilities:

Coordinate patient information, staff availability, and home health or hospice staffing with the intake department. Communicate referral sources and staff on home health or hospice services and regulatory requirements.

Gather patient demographic and medical information, treatment, and services to develop a comprehensive home health or hospice care referral and plan. Identify the type of service best suited for the referral patient and communicate this to the referral source and patient/family/caregiver.

Requirements:

Education:

  • Graduation from a state-accredited nursing school with a BSN preferred.
  • Enrollment in an accredited program within four years of employment and obtaining a Bachelor's degree in Nursing or a Master's degree in Nursing within seven years.

Licensure/Certification:

  • Licensed to practice as a Registered Nurse in North Carolina.

Experience:

  • Two years of home health, public health, or other health-related experience.

Salary: $49.81 per hour, with pay offers determined by experience and internal equity.

This role is employed by NC Health, a private subsidiary of UNC Heath Care System, and is exempt from overtime.
