Responsibilities- Provide one-on-one care to pediatric patients in their homes, ensuring their physical, emotional, and developmental needs are met.
- Develop and implement individualized care plans in collaboration with the patient's healthcare team.
- Administer medications, perform medical procedures, and provide wound care as needed.
- Monitor patient progress, report any changes or concerns to the healthcare team, and make adjustments to care plans as necessary.
- Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
- RN/LVN with a current, valid license to practice as issued by the State Board of Nursing.
- Current CPR certification.
