Resident Services Coordinator

The Resident Services Coordinator will play a vital role in supporting the well-being of residents in our communities. This position requires a strong understanding of social services and the ability to connect residents with resources that promote housing success.
Key Responsibilities- Provide non-clinical case management and referral services to residents, securing a range of social services as needed.
- Assess and identify resident needs, developing and implementing individual healthy aging plans.
- Conduct initial assessments within 30 days of move-in and update assessments annually or as needed.
- Utilize and maintain accurate and timely documentation through AASC Online.
- Organize and coordinate on-site wellness and health improvement programs, events, and activities.
- Produce weekly reports and submit to Housing Administrator, immediate supervisor, and contractor's request.
- Conduct outreach and engagement, monitoring services periodically, and home visits when needed.
- Collaborate with Activities Coordinator to develop activities within and outside the site, designed to build community and provide opportunities for socialization and wellness.
- Maintain accurate and timely documentation on all resident demographics, issues, requests, incidents, interactions, and outcomes through online software.
- Attend staff meetings and Resident Services Coordinator on-going training and/or conference.
- Bachelor's degree in Social Work, Sociology, Psychology, Gerontology, or related field or equivalent experience.
- Experience with or knowledge of HUD regulatory requirements is a plus.
- Excellent written and oral communication required.
- Basic computer skills, including ability to navigate the internet, use Outlook, create Word and Excel documents, and work with a web-based database.
- Must complete annual online training through learning management system and off-site in-person trainings as required.
The work environment and characteristics described here are representative of those an employee encounters while performing the essential functions of this job. The work environment is an office environment, community meeting room, or a resident's apartment. The noise level in the work environment is usually moderate.
While performing the duties of this job, the employee is frequently required to stand, walk, sit, use hands to finger, handle, or feel; and to hear and speak with peers and residents. The employee is often required to move continually throughout the work day; maintain dexterity to pinch small objects; reach with hands and arms and stoop, kneel, crouch, climb, or crawl. The employee must occasionally lift and/or move up to 25 pounds.
Able to drive 30% of the time to residents' apartments throughout the region (mileage reimbursed at current federal rate). Able to tolerate possible exposure to fumes, dust, and other environmental elements.
