Position Overview:
We are seeking a Patient Care Manager to become a vital part of our team.

Gentiva Hospice is expanding

This role is for a newly established Gentiva location.

Clinical Nursing Manager - Nursing Manager - Hospice - Full-Time

Standard Hours: Monday to Friday, 8 AM to 5 PM - Office-Based Role

Key Responsibilities:

  • Oversee hospice or home care case management
  • Lead and supervise nursing staff
  • BSN is preferred
  • Maintain an active RN License in the state of Virginia
You will report directly to the Executive Director, Administrator, or Senior Patient Care Manager.

Your responsibilities will include managing the daily operations of your designated hospice site, overseeing the clinical components of the hospice program, and ensuring high-quality care for patients.

In the absence of the Executive Director, Administrator, or Senior/Executive Patient Care Manager, you will manage the site.

Responsibilities include overseeing clinical, administrative, and recordkeeping functions.

Respond to phone referrals and inquiries promptly and compassionately.

Participate in the on-call nursing schedule and provide direct skilled nursing care in patients' homes.

Candidate Profile:

Education and Experience:
Registered Nurse with a minimum of an Associate's Degree in Nursing from an accredited institution. This requirement may be superseded by state-specific regulations.

A Bachelor's Degree in Nursing is preferred from an accredited institution.

At least three years of direct patient care experience in a clinical environment is required, unless otherwise specified by state regulations.

A minimum of one year of management or supervisory experience is essential.

Two or more years of experience in hospice or home care is required.

Current nursing license in the state of employment is mandatory.

Licenses, Certifications, and Registrations:
Current nursing license in the state of employment

CPR Certification

Valid driver's license and current automobile insurance

Personal Attributes:
Demonstrate sensitivity to the needs of terminally ill patients and their families before and after the patient's passing.

Ability to adapt to various tasks seamlessly, maintaining efficiency and composure.

Communicate effectively with individuals from diverse socioeconomic backgrounds. Influence perspectives and attitudes regarding care and services. Make informed decisions based on sound judgment. Exhibit strong patient advocacy. Display professionalism, maturity, cooperation, flexibility, and tact in interactions with colleagues and all members of the hospice team.

Specialized Knowledge and Skills:


Understanding of the needs of terminally ill patients and their families, along with a solid grasp of hospice concepts; familiarity with the roles of various disciplines providing hospice services; excellent patient assessment abilities; strong oral and written communication skills; comprehensive knowledge of managed care principles and regulatory guidelines (e.g., Medicare, Medicaid, and human resource management principles).
