Key Responsibilities:

  1. Facilitates the planning of services for clients by conducting interviews, assessing their needs and objectives, assisting in application processes, and connecting them with the service system to fulfill basic human necessities; organizes suitable transportation.
  2. Informs clients by clarifying program requirements, available options, interpretations, and decisions.
  3. Oversees service provision through follow-up interviews with clients and/or service providers.
  4. Delivers emergency and crisis intervention services by directing clients to other suitable programs.
  5. Maintains client documentation by recording their situations and actions; accurately completes necessary paperwork.
  6. Adheres to federal, state, and local legal standards by reviewing existing and new legislation; keeps management informed of required actions.
  7. Safeguards client confidentiality and protects operations by maintaining confidentiality in accordance with established policies and procedures; upholds professional boundaries with clients.
