Job Description

Job Title: SSVF Housing Navigator - Jacksonville

Job Summary:

We are seeking a highly motivated and compassionate Housing Navigator to join our team at Changing Homelessness, Inc. in Jacksonville. As a Housing Navigator, you will play a critical role in ending homelessness in our community by facilitating the rapid housing placement of Veterans experiencing homelessness. You will work closely with clients, SSVF Case Managers, and VASH Social Workers to assess housing preferences and needs, identify new housing opportunities, and provide support throughout the housing process.

Key Responsibilities:

  • Conduct housing assessments with clients to understand their housing preferences and needs
  • Identify new housing opportunities and work with landlords and property managers to secure placements
  • Provide support and guidance to clients throughout the housing process, including transportation to housing opportunities and assistance with move-in preparations
  • Collaborate with SSVF Case Managers and VASH Social Workers to ensure a comprehensive approach to housing placement
  • Develop and maintain relationships with community partners and stakeholders to leverage resources and support for clients

Requirements:

  • Associate's degree in a related field or equivalent work experience
  • Successful completion of a Level II background check
  • Valid driver's license and clean 3-year driving history
  • Ability to work independently and as part of a team
  • Strong communication and interpersonal skills

Preferred Qualifications:

  • Experience working with homeless populations and/or Veterans
  • Knowledge of community resources and funding sources
  • Ability to work in a fast-paced environment and adapt to changing priorities
