Job Title: Community Health Worker

At Millennium Physician Group, we are committed to providing exceptional healthcare services to our patients. We are seeking a highly motivated and compassionate Community Health Worker to join our team.

Job Summary

The Community Health Worker will play a vital role in bridging the gap between healthcare providers, community services providers, and individuals in the community to promote health, reduce disparities, and improve service delivery.

Responsibilities
  • Health Resource Navigation: Work with Public Health Specialists, Health Educators, Case Managers, Social Workers, Primary Care staff, and community agencies to link patients to needed preventative health services.
  • Health Education: Provide health education to MPG patients regarding the primary care medical home and available resources such as the Virtual Care Clinic, Walk-in Clinics, and numerous value-based service offerings available to Millennium patients.
  • Community Resource Linkage: Link individuals to community resources to meet basic needs that influence health, such as housing, food, job placement, financial assistance, and literacy assistance.
  • Cultural Competence: Ensure that health care and social service systems are culturally relevant and responsive to their service population.
  • Health Coaching: Provide informal health coaching and support to patients.
  • Community Events: Staff tables at community events and assist patients with applications and paperwork.
Qualifications
  • Education: High school diploma; Bachelor's degree in social work or a related field is preferred.
  • Experience: 3+ years of experience as a Community Health Worker or in a similar role, preferred.
  • Cultural Competence: Experience working with people of different cultures and socio-economic backgrounds.
  • Healthcare Knowledge: Understanding of the healthcare system and how to help patients find necessary resources.
Physical Demands

The Community Health Worker will work in a fast-paced, cross-functional environment and will be required to spend 80% of their time doing outreach, health education, and navigation in community settings.
