About the Role

We are seeking a skilled Certified Wound Ostomy RN to join our team at Providence Home Services Oregon. As a key member of our home health team, you will provide high-quality patient care and support to individuals in their homes, focusing on wound and ostomy management.

Responsibilities
  • Provide direct and indirect quality care for patients of all ages, following the nursing process and in accordance with the Nurse Practice Act.
  • Accept professional development as an integral part of nursing practice.
  • Work collaboratively with patients, families, and healthcare teams to develop and implement individualized care plans.
  • Conduct thorough assessments, develop and implement care plans, and provide education to patients and families on wound and ostomy management.
  • Collaborate with other healthcare professionals to ensure seamless care transitions and coordination.
Requirements
  • Bachelor's Degree in Nursing from an accredited school of nursing.
  • Completion of an accredited course in Wound, Ostomy and Continence.
  • National Provider BLS - American Heart Association certification within 30 days of hire.
  • Oregon Registered Nurse License upon hire.
  • Washington Registered Nurse License upon hire (if practicing within this state).
  • Certification as a Enterostomal Therapist by the Wound Ostomy and Continence Nursing Certification Board within 90 days of hire.
  • 2 years of nursing experience.
Preferred Qualifications
  • 1 year of home care experience (either prior to or upon certification as ET).
