Job Summary

We are seeking a highly skilled and compassionate Senior Care Manager to oversee the daily operations of our in-home care services. As a key member of our team, you will be responsible for managing a team of caregivers, coordinating care plans, and ensuring compliance with healthcare regulations.

Key Responsibilities
  • Develop and Implement Care Plans: Create personalized care plans for senior clients based on their needs and healthcare requirements.
  • Supervise and Manage Caregivers: Supervise and manage a team of caregivers, ensuring they follow care plans and provide high-quality care.
  • Conduct Regular Assessments: Conduct regular assessments of clients' health status and adjust care plans as necessary in collaboration with healthcare providers.
  • Coordinate Client Care: Coordinate and communicate effectively with clients, families, and healthcare professionals to provide updates on client care and address any concerns or issues.
  • Ensure Compliance: Ensure compliance with state and federal healthcare regulations, as well as company policies and procedures.
  • Train and Develop Caregivers: Conduct training and development sessions for caregivers, ensuring they are equipped with the skills needed to provide excellent care.
  • Recruit and Onboard Caregivers: Participate in the recruitment and on-boarding of new caregivers, including conducting interviews and background checks.
  • Schedule Client Visits: Schedule client visits and caregiver shifts, ensuring all client care needs are met in a timely manner.
  • Provide Hands-on Care: Provide hands-on care as needed, particularly for clients with more complex medical needs.
  • Maintain Accurate Records: Maintain accurate client records using electronic medical records (EMR) systems and ensure confidentiality of all medical and personal information.
  • Support Caregivers: Act as a resource for caregivers, providing guidance and support for challenging client cases.
  • Monitor Quality of Care: Monitor the quality of care provided, conducting client satisfaction surveys and making necessary improvements to enhance service delivery.
Qualifications
  • Licensed Practical Nurse (LPN): Current LPN license in the state of [State].
  • Experience in Home Care: Minimum of 2-3 years of experience in a clinical or home care setting, with experience in case management or client care coordination preferred.
  • Leadership Experience: Previous experience managing a team of caregivers or healthcare professionals is a plus.
  • Knowledge of Senior Care: Knowledge of senior care services, including personal care, dementia care, and chronic illness management.
  • Regulatory Knowledge: Strong understanding of state and federal healthcare regulations, particularly those related to home care and eldercare.
  • Technical Skills: Proficiency in using electronic medical records (EMR) and scheduling software.
  • Communication Skills: Excellent communication and interpersonal skills with the ability to build rapport with clients, families, and healthcare providers.
  • Problem-Solving Skills: Ability to work independently and manage multiple priorities in a fast-paced environment.
