Senior Care Manager

The Senior Care Manager will oversee the daily operations of in-home care services, ensuring high-quality care for senior clients. This role involves managing a team of caregivers, coordinating care plans, and maintaining compliance with healthcare regulations.
Key Responsibilities:
- Develop and implement personalized care plans for senior clients based on their needs and healthcare requirements.
- Supervise and manage a team of caregivers, ensuring they follow care plans and provide high-quality care.
- Conduct regular assessments of clients' health status and adjust care plans as necessary in collaboration with healthcare providers.
- Coordinate and communicate effectively with clients, families, and healthcare professionals to provide updates on client care and address any concerns or issues.
- Ensure compliance with state and federal healthcare regulations, as well as company policies and procedures.
- Conduct training and development sessions for caregivers, ensuring they are equipped with the skills needed to provide excellent care.
- Participate in the recruitment and on-boarding of new caregivers, including conducting interviews and background checks.
- Schedule client visits and caregiver shifts, ensuring all client care needs are met in a timely manner.
- Provide hands-on care as needed, particularly for clients with more complex medical needs.
- Maintain accurate client records using electronic medical records (EMR) systems and ensure confidentiality of all medical and personal information.
- Act as a resource for caregivers, providing guidance and support for challenging client cases.
- Monitor the quality of care provided, conducting client satisfaction surveys and making necessary improvements to enhance service delivery.
- Current Licensed Practical Nurse (LPN) license.
- Minimum of 2-3 years of experience in a clinical or home care setting, with experience in case management or client care coordination preferred.
- Previous experience managing a team of caregivers or healthcare professionals is a plus.
- Knowledge of senior care services, including personal care, dementia care, and chronic illness management.
- Strong understanding of state and federal healthcare regulations, particularly those related to home care and eldercare.
- Proficiency in using electronic medical records (EMR) and scheduling software.
- Excellent communication and interpersonal skills with the ability to build rapport with clients, families, and healthcare providers.
- Ability to work independently and manage multiple priorities in a fast-paced environment.
