Case Manager

We are seeking a highly motivated and compassionate Case Manager to join our team at New Directions for Veterans. As a Case Manager, you will play a critical role in providing individualized support and services to veterans, helping them achieve their goals and improve their overall well-being.
Responsibilities- Provide rehabilitative services to veterans, focusing on improving their functioning and overall quality of life.
- Link veterans to community resources, such as housing, medical, and financial services, to support their recovery and well-being.
- Accompany veterans to appointments and community events to enhance their access to resources and support.
- Collaborate with multidisciplinary treatment teams to develop and implement individualized treatment plans.
- Maintain accurate and timely clinical records, adhering to Los Angeles County Department of Mental Health guidelines.
- Provide crisis intervention and support to veterans in need.
- Perform other duties as assigned to support the team and organization.
- Demonstrated experience working in the mental health field, with a focus on rehabilitation and support services.
- Compassion and empathy for veterans, with a commitment to making a positive impact in their lives.
- Ability to work effectively with a diverse population, including veterans with severe and persistent mental illnesses.
- Strong written and verbal communication skills, with the ability to communicate effectively with veterans, colleagues, and community partners.
- Comfort working in a fast-paced environment, with the ability to prioritize tasks and manage time effectively.
- Ability to work independently and as part of a team, with a focus on collaboration and mutual support.
- Proficiency in basic office software, including word processing, spreadsheets, and data collection programs.
This position requires the ability to work in a dynamic and sometimes challenging environment, interacting with veterans who may be experiencing crisis or trauma. The successful candidate will be able to maintain a high level of concentration and attention to detail, while also being able to communicate effectively and work collaboratively with others.
The physical demands of this position include standing, sitting, walking, and using hand and finger dexterity to perform tasks. The ability to lift and carry up to 25 pounds may be required on occasion.
