Mission Pediatrics is seeking a highly skilled Behavioral Health Manager to join our team. As a Behavioral Health Manager, you will play a crucial role in providing high-quality care management to improve the overall health and well-being of pediatric members and their families.

Job Summary:

The Behavioral Health Manager will be responsible for managing a caseload of members with behavioral health needs, utilizing brief behavioral health therapeutic interventions to improve the member's ability to manage their own health, and providing formal and informal training and support for ECM members on behavioral health conditions.

Key Responsibilities:
  • Manage a caseload of members with behavioral health needs
  • Utilize brief behavioral health therapeutic interventions to improve member health
  • Provide formal and informal training and support for ECM members on behavioral health conditions
  • Assist with the coordination of medical and behavioral health access issues with PCP offices, specialists, and ancillary services
  • Track medical and behavioral health outcome measures in a patient registry
  • Ensure smooth transitions of care, coordination with psychiatric hospitals for M1 and with IEHP transitions of care team for M2, regarding client admission/discharges
  • Collaborate on member care issues with other ECM care team members, participating in weekly systematic case reviews and ad hoc case reviews
  • Work with members to identify health/wellness goals and incorporate these goals into the member's shared care plan
  • Aid Nurse Care Manager in overall review of member comprehensive health assessments (CHA) upon completion by another care team member
  • Provide consultation for all members of the team when discussing tasks of a clinical nature
  • Demonstrate the highest ethical behavior in relationships with co-workers, supervisors, members, providers, and community colleagues
  • Exhibit dedication to continuous quality improvement by actively participating in initiatives and projects
  • Ensure accurate documentation, adhering to regulatory requirements and accreditation standards
Requirements:
  • Master's degree in social work or Marriage and Family Therapy (MSW or MFT) from an accredited institution
  • Possession of an active, unrestricted, and unencumbered Clinical Social Worker (LCSW) or Marriage and Family Therapist (LMFT) license issued by the California Board of Behavioral Sciences
  • Registered with the Board of Behavioral Sciences
  • Minimum of three (3) years of experience performing or facilitating Behavioral Health services in a health clinic/psychiatric hospital medical facility
