Job Title: Hospice Admissions LVN

About the Role:

The Admissions LVN works under the supervision of an RN to provide professional nursing care to patients across the lifespan, adhering to established standards and practice. This role requires collaboration with the admissions team to ensure timely and efficient service delivery.

Responsibilities:

  • Assists RN in analyzing patient situations and taking appropriate actions.
  • Evaluates patient responses to care and documents outcomes in a timely manner.
  • Implements and documents teaching plans based on patient and family needs.
  • Delivers services under the supervision of an RN and in collaboration with the healthcare team.
  • Documents all care provided to patients, including telephone interactions with staff members.
  • Enters or retrieves data from established computer files.
  • Orients, instructs, and trains assigned personnel.
  • Performs phlebotomy as needed.
  • Provides support to the agency in completing assigned audits and office duties.

Requirements:

  • Minimum 12 months experience as an LVN.
  • Preferred experience in acute care, home health, or hospice.
  • Current California LVN license.
  • CPR certification and negative TB skin test.
  • Ability to answer phones, maintain patient charts, and schedule appointments.
  • Phlebotomy certification preferred.
  • CHPLN certification preferred.
  • Spanish speaking may be preferred or required.
  • Current California driver's license and proof of auto insurance required.
