Compensation Package:
Annual salary range: $98,000 - $130,800

Key Responsibilities:
- Utilize an interdisciplinary approach to evaluate the medical, physical, social, emotional, and spiritual needs of patients, ensuring families receive appropriate support in line with the patient's care plan.
- Engage in team meetings, interdisciplinary management discussions, family consultations, and patient interactions, either in-person or via phone.
- Ensure compliance with regulatory standards through regular audits and monitoring of processes and documentation.
- Collaborate with Quality Improvement and Education teams to uphold professional practice standards and participate in the development of quality initiatives and educational programs.
- Coordinate and analyze clinical, regulatory, and financial practices to support organizational goals and improve hospice care delivery.
- Oversee daily operations, ensuring effective management of caseloads and achievement of productivity and financial targets.
Qualifications:
Licenses and Certifications:
- Current registration as a Registered Professional Nurse in New York State is required.
Education:
- Bachelor's degree in nursing from an accredited program or equivalent experience is required.
Experience:
- A minimum of three years of clinical nursing experience, preferably in hospice or home care settings, is required.
- Experience in clinical management is preferred.
