Position: Physical Therapist (PT)

The Trevecca Center for Rehabilitation and Healing is currently seeking a skilled Physical Therapist to join our dedicated team. This role involves immediate engagement in providing high-quality rehabilitation services.

Key Responsibilities:


The Physical Therapist will be responsible for evaluating patients referred for rehabilitation. This includes:
- Delivering direct patient care to assess medical conditions, functional abilities, limitations, and rehabilitation potential.
- Developing and implementing tailored treatment plans with specific objectives based on each patient's capabilities and tolerance, under the guidance of a physician.
- Setting realistic and achievable goals for patients, meticulously documenting their progress, and educating both patients and their families on effective physical therapy techniques.


Qualifications:
- Graduation from an accredited Physical Therapy program.
- Possession of a valid professional license and registration in the state of practice.
- Oversight and supervision of Physical Therapy Assistants.
- Preferred experience in long-term care and/or geriatric rehabilitation.
- Proficiency in documentation, including evaluations, progress notes, recertifications, and discharge summaries.
- Collaboration with multidisciplinary teams to ensure optimal patient-centered care.
- Strong patient service skills and a commitment to clinical excellence.
- Punctuality, adherence to scheduled hours, and professionalism are essential.
- Compliance with confidentiality, HIPAA, and federal residents' rights regulations.
- Familiarity with Case Mix Index and PPS scheduling is advantageous but not mandatory.
