About the Role:

The Hospital at Home Operations Coordinator will play a vital role in supporting the administrative and logistical functions of our Hospital at Home program. This position requires strong organizational skills, attention to detail, and the ability to work effectively in a remote patient-facing environment.

Key Responsibilities:

  • Support Hospital at Home leadership and nurse staffing with organizing patient assignments and scheduling of all virtual and in-person care team visits.
  • Coordinate supply and equipment delivery to patients' homes and arrange patient transportation as needed.
  • Work closely with the interdisciplinary team to ensure seamless patient care operations.

Requirements:

  • Previous administrative support experience in a healthcare setting or direct patient care experience required.
  • Strong customer service skillset and detail-oriented.
  • High School Diploma required; Associates Degree preferred.
