Advanced Practice Provider

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at Oak Street Health. As a key member of our care team, you will provide exceptional primary care to our patients, working collaboratively with physicians and other healthcare professionals to deliver high-quality, patient-centered care.
Key Responsibilities:
Provide routine and acute care to patients, including conducting office visits, administering wellness visits, and coordinating care with other providers and specialists.
Develop and implement individualized care plans, taking into account patients' unique needs and health goals.
Collaborate with the care team to ensure seamless care coordination and population health leadership.
Educate patients on their health conditions, care plans, and promote healthy behaviors.
Participate in quality improvement initiatives and contribute to the development of best practices in primary care.
Requirements:
Master's degree in Nursing or related field.
National certification in a relevant specialty (e.g., Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner).
Active, non-probationary state Nurse Practitioner license.
DEA license.
US Work Authorization.
Preferred Qualifications:
Experience in primary care, internal medicine, geriatrics, or family medicine.
Understanding of HCC documentation, ICD-10 coding, and Health Risk Assessments (HRAs).
Passion for teamwork and collaboration.
Bilingual proficiency in applicable areas.
