What We're Looking For

We are seeking a skilled and compassionate Primary Care Nurse to join our team at the Community Health Center in Enfield, Connecticut. As a key member of our clinical staff, you will provide high-quality, patient-centered care to individuals and families throughout the lifespan.

About the Ideal Candidate:

The ideal candidate will have a Bachelor of Science in Nursing (BSN) degree and a valid nursing license. You will possess excellent communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.

Responsibilities:

  • Provide general and specific health care education to patients
  • Work with patients to identify and monitor self-management goals
  • Collaborate with other community-based providers to ensure seamless care coordination
  • Follow up on emergency room visits and hospital discharges
  • Assist providers with exams, treatments, and procedures as needed

Compensation:

The estimated salary for this position is $85,000 - $110,000 per year, depending on experience. We offer a comprehensive benefits package, including health insurance, 403B, tuition reimbursement, and paid time off.
