**Job Title:** Patient Care Coordinator
Estimated Salary: $40,000 - $60,000 per year
Parkway Pavilion Health & Rehabilitation Center, a leading short-term rehab and skilled nursing facility in Enfield, CT, is seeking an experienced Certified Nursing Assistant to join our team as a Patient Care Coordinator.
This role involves providing direct patient care under the supervision of licensed nurses, assisting with daily tasks, and maintaining a safe and clean environment for residents. As a valued member of our healthcare team, you will play a critical role in ensuring optimal patient outcomes and contributing to our mission of delivering exceptional care.
Key Responsibilities:
- Assist with personal care activities, such as bathing, dressing, and grooming
- Provide emotional support and companionship to residents
- Document patient information and maintain accurate records
- Collaborate with healthcare teams to develop and implement individualized care plans
- Perform light housekeeping duties and maintain a clean and organized environment
Requirements:
- Current C.N.A. certification in CT or MA
- Ability to work varied shifts, including weekends
- Excellent communication and interpersonal skills
- Capacity to lift 25 pounds and stand for long periods
