Primary Care Physician Assistant

At Lyle Health, we are seeking a dedicated and skilled Primary Care Physician Assistant to join our team in Enfield, CT. This is a full-time position offering a unique blend of clinical responsibility and administrative support.
About the Role:
As a Primary Care Physician Assistant at Lyle Health, you will be responsible for delivering high-quality patient care in an outpatient setting. Your primary focus will be on providing direct patient care, including medical histories, physical examinations, diagnosis, and treatment plans. Additionally, you will participate in hospital rounds, coordinate patient care, and maintain accurate medical records.
Key Responsibilities:
* Provide direct patient care, including history taking, physical examinations, diagnosis, and treatment planning
* Participate in hospital rounds and develop individualized care plans
* Coordinate patient care with other healthcare professionals and provide education to patients and families
* Maintain accurate and up-to-date medical records
* Collaborate with physicians and other healthcare professionals to ensure seamless patient care
Requirements:
* Board certification as a Nurse Practitioner or Physician Assistant required
* Prior experience in primary care, family practice, or a related field
* Active Connecticut state license
* Epic experience and strong technical skills
* Excellent communication and interpersonal skills
