About the Role:
We are seeking an experienced Licensed Clinical Social Worker to join our team. As a Behavioral Health Clinician, you will provide comprehensive mental health services to patients in skilled nursing facilities, including assessment, diagnosis, treatment planning, and implementation of individualized care plans. You will work closely with our multidisciplinary team to ensure seamless care coordination and delivery. The ideal candidate will have a strong background in geriatric mental health, excellent communication skills, and a passion for delivering patient-centered care. Estimated salary: $85,000 - $110,000 per year.

Responsibilities:
- Provide comprehensive mental health assessments and diagnoses to patients in skilled nursing facilities.
- Develop and implement individualized care plans tailored to each patient's needs.
- Collaborate with our multidisciplinary team to ensure seamless care coordination and delivery.
- Conduct regular follow-up sessions to monitor patient progress and make adjustments to care plans as needed.
- Maintain accurate and timely documentation of patient interactions and care plans.

Requirements:
- Licensed Clinical Social Worker (LCSW) or equivalent experience.
- Strong background in geriatric mental health.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
- Strong problem-solving and critical thinking skills.
