Job Title: Care Management Supervisor

We are seeking a highly skilled Care Management Supervisor to join our team at The Sparc Network. As a Care Management Supervisor, you will be responsible for providing clinical, administrative, and operational supervision and guidance to our Care Management team.

Key Responsibilities:
  • Supervise a team of Care Managers to ensure they are meeting expectations, requirements, and timelines in areas such as care management comprehensive assessment, care plans, and individual support plans.
  • Develop and maintain relationships with clinical consultants to provide expertise and support to the care team.
  • Oversight of specific care management functions, including care coordination, transitional care management, diversion, and ensuring minimum contact requirements are met based on member acuity.
  • Provide oversight and management to the daily operations of care management, including reviewing and approving incident reports, progress notes, and other documentation.
  • Participate as a member of our agency's leadership team and contribute to the development and implementation of our care management program.
Requirements:
  • A license, provisional license, certificate, registration, or permit issued by the governing board regulating a human service profession (including LCSW, LMFT, LCAS, LCMHC, LPA, or RN).
  • Three years of experience providing care management, case management, or care coordination to the population being served.
