Nurse Care Specialist

We are seeking a compassionate and skilled Registered Nurse to join our hospice team at United Hospice. As a Registered Nurse, you will have the opportunity to provide high-quality care to patients and their families, making a meaningful difference in their lives.
Responsibilities:- Provide direct care to patients, utilizing accepted standards of nursing practice and facility policies and procedures.
- Administer medications and treatments according to established policy and procedure, and document all interventions accurately.
- Perform thorough and accurate patient assessments, and provide appropriate follow-up and documentation of all findings and interventions.
- Conduct the admission process for new patients, and participate in hospice-sponsored in-service training as required.
- Generate the initial plan of care, and perform ongoing assessments and care plan updates in collaboration with the Interdisciplinary Team.
- Maintain proficiency in working with technical equipment, such as feeding pumps, medication pumps, and suction machines.
- Collaborate and coordinate effectively with clinical and support staff to conduct daily patient care.
- Inform the physician and other staff of changes in patient needs and outcome of interventions.
- Document assessments, interventions, patient responses to treatments, and all communications/conferences/IDT meetings, care plans, and discharges.
- Valid and current NYS Registered Nurse license is required; BSN preferred.
- Two years recent clinical nursing experience with strong clinical skills, including one year recent Med-Surg experience, required.
- Complies with accepted nursing professional standards and practices.
- Commits to take and pass the hospice and palliative care nursing certification exam (CHPN) within three years of hire, if not currently certified.
- Commitment to the Hospice Philosophy of Care.
- Ability to relate to a diverse population and workforce in a personable and effective manner.
- Able to communicate effectively, while sustaining a caring, kind, and professional disposition.
- Must have excellent written and verbal communication skills.
- Proficient in MS Office Suite, EMRs, and ability to learn new software programs.
- Hospice admission and end-of-life care experience preferred.
- Must have daily access to a vehicle, valid driver's license/acceptable driving record, and auto insurance to travel to patient locations.
- Flexible schedule with weekend rotation as needed.
