Soleo Health is seeking a skilled Infusion Nurse to provide high-quality patient care in the comfort of their own homes. As a key member of our team, you will be responsible for delivering infusion therapy and other nursing services to patients in need.
Responsibilities:- Provide direct patient care, including assessments, care planning, and education on infusion therapy and other nursing services.
- Evaluate and educate patients and their caregivers on safe administration of home infusion therapy.
- Assess the home environment and identify interventions to ensure safe and effective care delivery.
- Develop and maintain patient care plans in collaboration with other clinical team members.
- Discharge patients and complete discharge summaries as needed.
- Manage caseload scheduling and communications with patients and healthcare teams.
- Bilingual proficiency in English and Spanish.
- Pediatric experience and a strong background in infusion therapy.
- RN degree, BSN preferred.
- Advanced certification in specialty areas of practice (e.g., CRNI, ONS, IgRN) preferred but not required.
- Availability for on-call rotations, including nights and weekends, to address urgent issues and ensure continuous operations.
- Administrative oversight, including workflow monitoring, policy compliance, and resource management.
- Emergency availability to respond promptly to emergencies and ensure rapid resolution of issues.
- Minimum 3 years of nursing experience with a background in infusion therapy.
- Current nursing license for applicable home state.
- Experience in ICU and/or Cardiac Care preferred.
- Proficiency in Microsoft Outlook and Microsoft Office Suite.
- Valid state-issued driver's license with no significant restrictions.
