Responsibilities:

• Obtain and interpret patient information to develop effective treatment plans
• Conduct patient education sessions and psychosocial evaluations as assigned
• Develop and implement service plans for assigned residents in a timely manner
• Assist with discharge planning and coordinate with other healthcare professionals
• Plan and lead group therapy and individual treatment sessions
• Maintain confidentiality and adhere to patient confidentiality policies

Requirements:

• Bachelor's or master's degree in a related field (CASAC or QHP required)
• 2 years' experience working in an addiction or behavioral health related field
• Strong understanding of multicultural issues and experience working with diverse populations
