RN Home Care Coordinator

Job Summary:
Empath Health seeks a skilled RN Home Care Coordinator to coordinate and facilitate nursing activities, including care planning and implementation, preparation for interdisciplinary/daily meetings, family conferences, and related follow-up.
The ideal candidate will adhere to all Empath Policies and Procedures, Empath Safety Policies and Procedures, and OSHA safety guidelines. They will assist staff in coordinating activities related to nursing activities, including assessment and care plan review meetings.
Key Responsibilities:
- Coordinate and facilitate nursing activities, including care planning and implementation
- Prepare for interdisciplinary/daily meetings, family conferences, and related follow-up
- Adhere to all Empath Policies and Procedures, Empath Safety Policies and Procedures, and OSHA safety guidelines
- Assist staff in coordinating activities related to nursing activities, including assessment and care plan review meetings
- Monitor operations to ensure consistent compliance with established quality assurance standards
- Responsible for interviewing, hiring process, disciplinary action, and annual performance appraisals
- Organize a structured schedule for new employees to be introduced to all departments within the PACE program and to complete initial job competencies
- Monitor employee fulfillment of job description and compliance with Empath Health, state, and federal regulations
Requirements:
- Graduate of a school of professional nursing program required
- Bachelor of Science Registered Nursing (BSRN) strongly preferred
- Current Florida Registered Nurses License and Cardiopulmonary Resuscitation (CPR) certification required
- A minimum of five (5) years of health care experience, and three (3) years of management experience, preferably in a geriatric care setting required
- A minimum of one year's experience in working with the frail or elderly population
- Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact
- Must have a valid driver's license, proof of insurance, and have means of transportation
