RN Case Manager Hospice Care Specialist

We are seeking a compassionate and skilled RN Case Manager to join our team at Assured Hospice of Port Angeles, a part of LHC Group. As a Hospice RN Case Manager, you will play a vital role in delivering high-quality end-of-life care to our patients and their families.
About the RoleAs a Hospice RN Case Manager, you will be responsible for:
- Conducting initial nursing evaluations to determine eligibility for hospice services
- Identifying patients' physical, psychosocial, and environmental needs and reassessing as needed
- Developing and implementing individualized plans of care in collaboration with the patient, family, and healthcare team
- Providing ongoing support and education to patients and families
To be successful in this role, you will need:
- One year of clinical experience as an RN
- Current RN licensure in the state of practice
- Current CPR certification
- Access to a dependable vehicle or public transportation
