RN Case Manager Hospice Specialist

We are seeking a skilled RN Case Manager to join our Hospice team at Assured Home Health & Hospice. As a Hospice RN, you will play a vital role in delivering high-quality end-of-life care to our patients and their families.
Key Responsibilities- Conduct initial nursing evaluations to determine eligibility for hospice services within 48 hours of referral.
- Identify patients' physical, psychosocial, and environmental needs, reassessing as needed every 15 days.
- Develop and coordinate individualized plans of care, maintaining accurate clinical notes and documenting patient progress.
- Collaborate with patients, families, attending physicians, and interdisciplinary teams to provide comprehensive care.
- One year of clinical experience as a Registered Nurse.
- Current RN licensure in the state of practice and one year of clinical experience.
- Current CPR certification and a valid driver's license with vehicle insurance.
