RN Hospice Case Manager

We are seeking a compassionate and skilled RN Case Manager to join our hospice team at Assured Hospice of Port Angeles, a part of LHC Group. As a Hospice RN, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.
About the RoleAs a Hospice RN Case Manager, you will be responsible for delivering high-quality end-of-life care to our patients. This includes conducting initial nursing evaluations, identifying patient needs, and developing and implementing individualized plans of care. You will also collaborate with our interdisciplinary team to ensure seamless care coordination and excellent patient outcomes.
Responsibilities- Conduct initial nursing evaluations to determine patient eligibility for hospice services
- Identify patient needs and develop individualized plans of care
- Collaborate with the patient, family, and healthcare team to ensure high-quality care
- Document patient progress and outcomes
- Current RN licensure in the state of practice
- One year of clinical experience
- Current CPR certification
- Valid driver's license and access to a dependable vehicle
