Key Responsibilities- Initial Nursing Evaluation: Conduct thorough assessments of patients to determine eligibility for hospice services within 48 hours of referral.
- Plan of Care Development: Collaborate with patients, families, and healthcare teams to create personalized plans of care that address physical, psychosocial, and environmental needs.
- Continuous Care Coordination: Ensure seamless care transitions and coordinate with healthcare providers to ensure patients receive comprehensive support.
- Accurate Documentation: Maintain accurate and detailed clinical notes, documenting patient progress, goals, interventions, and responses to care.
- Interdisciplinary Collaboration: Work closely with patients, families, attending physicians, and other healthcare professionals to provide holistic care and support.
- Experience: One year of clinical experience as an RN.
- Licensure: Current RN licensure in the state of practice and one year of clinical experience.
- Certifications: Current CPR certification and a valid driver's license with vehicle insurance.
