POSITION SUMMARY:

The Clinical Nurse II is a registered nurse who takes on responsibility and accountability as outlined by the standards of nursing practice. This role involves being a full partner within the healthcare team, consistently demonstrating safe and effective nursing practices within the organizational framework.

The Clinical Nurse II possesses the essential knowledge, skills, and attitudes required to meet the standards for proficient nursing practice. This position reflects the mission of Penn Medicine and incorporates the professional practice competency domains. The Clinical Nurse II independently delivers patient care through the application of the nursing process, taking accountability for the care of assigned patients, which may include individuals from infancy to the elderly.

The Clinical Nurse II exemplifies the competency domains of Continuous Quality Improvement, Evidence-Based Practice, Leadership, Person and Family-Centered Care, Professionalism, Safety, Technology/Informatics, and Teamwork.

This role requires close collaboration with the interprofessional healthcare team to ensure coordinated care across the continuum. The nurse focuses on enhancing their knowledge and skills to provide personalized care that considers the physical, psychosocial, cultural, educational, safety, and age-related needs of patients.

The Clinical Nurse II actively seeks feedback while assuming leadership roles and contributes to improved clinical practices. The nurse demonstrates the impact of nursing practice on the organizational vision, mission, and goals, as well as the care delivery system. Compliance with all regulations and standards set by regulatory and accrediting bodies is essential.

ESSENTIAL FUNCTIONS: Qualified individuals must have the ability (with or without reasonable accommodation) to perform the following duties:

Integrate the mission, vision, and values of Lancaster General Health into patient care planning. Function as both an independent and interdependent member of the healthcare team as defined in the unit-based Plan for Provision of Care. Conduct patient assessments, focusing on physiological, psychological, and cognitive functions. Evaluate progress towards achieving outcomes and adjust the care plan based on recognized trends and changes. Collect and manage comprehensive data relevant to the patient's health. Analyze assessment data to determine diagnoses or issues. Develop age-appropriate and culturally sensitive care plans. Implement care knowledgeably and coordinate patient care across various disciplines and caregivers. Provide leadership to team members by effectively communicating expected standards of care and supporting their contributions. Demonstrate a commitment to the values of privacy, quality, respect, service, and teamwork. Assign, delegate, and supervise other staff, intervening as necessary to ensure the care plan is executed, continuity of care is maintained, and established outcomes are achieved through the effective use of resources. Educate patients about healthy practices and treatment options. Continuously enhance the quality and effectiveness of nursing practices, utilizing technology where appropriate. Consider factors related to safety, effectiveness, cost, and impact on practice when planning and delivering nursing services.

SECONDARY FUNCTIONS: The following duties are considered secondary to the primary duties listed above:

Manage rapidly changing situations by following established protocols, coordinating with appropriate health team members and resources, and documenting events. Engage in process improvement activities aimed at enhancing continuity in the nurse-patient relationship, minimizing redundancies, and seeking innovative solutions to current challenges. Complete mandatory education and training that includes a review of age-specific needs, caring behaviors, and interpersonal skills as defined by the unit-based Plan for Provision of Care. Maintain a safe working environment by adhering to established evidence-based practice policies and procedures and promptly reporting safety variances. Exhibit pride in work through positive interactions with staff, coworkers, patients, and all individuals encountered. Pursue experiences and learning activities to maintain and develop clinical and professional skills and knowledge. Provide supervision, guidance, counseling, and mentoring to peers and subordinates to enhance learning experiences and improve the professional practice environment. Participate in the employee performance process related to peers and subordinate team members; address employee issues, complaints, or requests as the first level of supervision; evaluate performance post-hire. Participate in on-call responsibilities as required. Other duties as assigned.

JOB REQUIREMENTS

MINIMUM REQUIRED QUALIFICATIONS:

RN licensure is required, issued by the PA Board of Nursing. Bachelor of Science in Nursing (BSN) or a higher-level nursing degree is required. For Magnet designated RN positions, a BSN or higher-level nursing degree is required for incumbents based on previously established parameters; or within two years of placement in position for incumbents who started after 07/01/. Completion of the Nurse Residency Program or more than 15 months of practice. CPR Certification - American Heart Association (AHA) Basic Life Support, formerly Healthcare Provider (AHA Heart Saver courses for CPR are not accepted) required within 30 days of hire. A healthcare provider in good standing with Medicare, Medicaid, and other federal and state health insurance programs, i.e., not excluded from participation in Medicare, Medicaid, or any other federal or state health insurance program.
