ROLE SUMMARY: The Clinical Nurse II is a registered nurse who takes on significant responsibility and accountability as outlined by the standards of nursing practice. This role requires collaboration as a key member of the healthcare team, consistently ensuring safe and effective practice within the established professional framework.
The Clinical Nurse II possesses the essential knowledge, skills, and professional demeanor to meet the benchmarks for competent nursing practice. This position exemplifies the experience standards reflective of the organization's mission, integrated within the professional competency domains. The Clinical Nurse II independently delivers patient care through the nursing process and is accountable for the care of assigned patients, spanning from infancy to the elderly.
The Clinical Nurse II demonstrates proficiency in Continuous Quality Improvement, Evidence-Based Practice, Leadership, Person and Family-Centered Care, Professionalism, Safety, Technology/Informatics, and Teamwork.
Collaboration with the interprofessional healthcare team is crucial to ensure coordinated care throughout the continuum. The nurse focuses on enhancing knowledge and skills necessary for providing personalized care, considering physical, psychosocial, cultural, educational, safety, and age-related factors for assigned patients.
The Clinical Nurse II actively seeks feedback in leadership roles and contributes to improving clinical practices. The nurse illustrates how nursing practices influence the organizational vision, mission, and goals, adhering to all regulatory and accreditation standards.
KEY RESPONSIBILITIES: Qualified candidates must be capable (with or without reasonable accommodation) of performing the following tasks:
- Integrate the mission, vision, and values of Lancaster General Health into patient care planning.
- Function as both an independent and collaborative member of the healthcare team as defined in the unit-based Plan for Provision of Care.
- Conduct comprehensive patient assessments, emphasizing physiological, psychological, and cognitive functions. Evaluate progress towards outcomes and adjust care plans based on observed trends and changes. Gather and manage detailed data relevant to the patient's health or situation.
- Analyze assessment data to identify diagnoses or issues. Develop age-appropriate and culturally sensitive care plans.
- Implement care knowledgeably and coordinate patient care across various disciplines and caregivers.
- Provide leadership to team members by clearly communicating expected care standards and supporting their contributions.
- Demonstrate a commitment to values of privacy, quality, respect, service, and teamwork.
- Delegate and supervise other staff members, ensuring the care plan is executed, continuity of care is maintained, and established outcomes are achieved through effective use of resources.
- Educate patients on healthy practices and treatment options.
- Continuously enhance the quality and effectiveness of nursing practices, utilizing technology as appropriate.
- Consider safety, effectiveness, cost, and impact on practice when planning and delivering nursing services.
- Manage rapidly changing situations by following established protocols, coordinating with appropriate health team members and resources, and documenting events.
- Engage in process improvement initiatives aimed at enhancing continuity in nurse-patient relationships, minimizing redundancies, and improving efficiency.
- Complete mandatory training that includes age-specific needs, caring behaviors, and interpersonal skills as defined by the unit-based Plan for Provision of Care.
- Maintain a safe working environment by adhering to evidence-based practice policies and promptly reporting safety variances.
- Exhibit pride in work through positive interactions with staff, colleagues, patients, and all stakeholders.
- Pursue experiences and learning opportunities to maintain and enhance clinical and professional skills.
- Provide supervision, guidance, and mentoring to peers and subordinates to improve the professional practice environment.
- Participate in performance evaluations for peers and subordinates, addressing issues as the first level of supervision.
- Participate in on-call responsibilities as required.
- Other duties as assigned.
MINIMUM REQUIRED:
- Current RN licensure issued by the PA Board of Nursing.
- Bachelor of Science in Nursing (BSN) or higher nursing degree is required. For Magnet designated RN positions, BSN or higher is required for incumbents based on established parameters; or within two (2) years of placement for those who started after specified date.
- Completion of the Nurse Residency Program or over 15 months of nursing practice.
- CPR Certification from the American Heart Association (AHA) Basic Life Support required within 30 days of hire.
- Must be a healthcare provider in good standing with Medicare, Medicaid, and other federal and state health insurance programs.
- As outlined in the unit-based Plan for Provision of Care.
