Job Summary

We are seeking a highly motivated and compassionate Income Maximization Case Manager to join our team at Volunteers of America Mid-States. As a key member of our Supportive Services for Veteran Families (SSVF) program, you will play a vital role in assisting veterans in ending their housing crisis, enhancing their independent living skills, and connecting them with community resources.

Responsibilities
  • Connect veterans with benefits, including SOAR applications, Veteran benefits, and other mainstream benefit sources.
  • Provide case management duties, including assessment, case plan development, and follow-up activities.
  • Establish linkages with appropriate agencies and service providers in the area/community.
  • Provide referrals and coordinate with VAMCs and other providers.
  • Connect all veterans with the American Job Centers and career centers.
  • Decide how resources are allocated to participants based on need.
  • Educate participants on supportive services availability and participant rights.
  • Provide referrals for supportive services to participants, including financial planning services, transportation services, income support services, fiduciary and representative payee services, legal services, childcare, and housing counseling.
  • Complete required documentation within 48 hours of contact and enter data into the Homeless Management Information System (HMIS).
  • Demonstrate good clinical judgment in decision making regarding participants.
  • Demonstrate ability to relate to veterans and their family in a culturally appropriate manner.
Requirements
  • Bachelor's degree in social work or related field and less than five (5) years of experience in the field.
  • At least five (5) years of experience in the field and no degree.
  • Or a Veteran with three (3) years of related work experience.
  • Demonstrated ability to provide a reasonable combination of family needs assessment, knowledge, and ability to access local resources, advocacy, counseling, and working cooperatively with local service providers.
  • Personal automobile, valid driver's license, liability insurance, and willingness to travel between counties up to 70% of the time.
  • Completion of required case management training within 90 days of hire and completion of SSI/SSDI Outreach, Access, and Recovery (SOAR) online training.
