The Hospice Registered Nurse is a vital member of our interdisciplinary team, responsible for delivering high-quality patient care and support to individuals and their families in the comfort of their own homes.
Key Responsibilities:
- Develop and implement individualized care plans that meet the unique needs and goals of each patient.
- Provide comprehensive nursing care, including symptom management, wound care, and medication administration.
- Collaborate with the healthcare team to ensure seamless care coordination and communication.
- Conduct thorough assessments and evaluations to identify patient needs and develop effective care plans.
- Document patient progress and outcomes in a timely and accurate manner.
Requirements:
- Graduate of an accredited school of nursing with a BSN degree preferred.
- Current licensure as a registered nurse (RN) in the State of Michigan.
- Minimum of two (2) years of nursing experience in a hospice or oncology setting.
- Current Michigan driver's license and/or reliable transportation.
